You can book your initial appointment by clicking the Book Now button. This will take you to the patient portal, where you can fill out the patient intake forms and book an initial appointment.

The Patient Portal offers a convenient, all-in-one platform for scheduling appointments, submitting patient information, managing payment options, and securely communicating with your provider.

Medication refills for controlled substances are regulated and can only be issued every 30 days. To ensure your refill is ready for pick up on day 30, we typically schedule follow-up appointments 28 to 29 days after your last visit; however, these appointments can be modified or planned through the patient portal or by phone to accommodate your needs.
